Why Use High Molecular Weight Hyaluronic Acid (HMWHA) Instead of Low Molecular Weight Hyaluronic Acid (LMWHA) During Professional Treatments, Especially Post-Needling?

Following tissue injury, monocytes can enter the tissue and differentiate into fibroblast-like cells called fibrocytes. However, the regulation of this differentiation is not fully understood.

The extracellular matrix contains high molecular weight hyaluronic acid (HMWHA; ∼2×10^6 Da). During injury, HMWHA breaks down into low molecular weight hyaluronic acid (LMWHA; ∼0.8–8×10^5 Da).

• HMWHA: Promotes the differentiation of human monocytes into fibrocytes and supports regenerative healing in adult wounds. Monocytes internalize HMWHA and LMWHA equally well.
• LMWHA: Inhibits fibrocyte differentiation and increases the expression of macrophage inflammatory protein-1α and monocyte chemotactic protein-1, suggesting its role in inducing the inflammatory cascade.
